Zealand Pharma and Roche said Wednesday that petrelintide, their amylin analog, met its primary endpoint in the Phase 2 ZUPREME-2 trial in people with overweight or obesity and type 2 diabetes. The 220 participants, all receiving metformin with or without an SGLT2 inhibitor and a baseline HbA1c of 8.0 percent, were randomized to three petrelintide doses or placebo alongside diet and activity guidance.
Weight loss at week 28 ranged from 7.4 to 9.2 percent across the three dose arms versus 2.0 percent for placebo, on the efficacy estimand, and all three arms beat placebo with statistical significance. On blood sugar, petrelintide delivered a placebo-adjusted HbA1c reduction of 0.61 to 0.88 percent. Tolerability was the standout feature: only 1.9 percent of petrelintide patients discontinued for gastrointestinal adverse events, essentially identical to the 1.7 percent placebo rate, a profile far cleaner than what GLP-1 drugs typically produce.
The cross-trial comparison is the sobering part. Eli Lilly’s amylin receptor agonist eloralintide posted up to 11.3 percent weight loss at 12 weeks in Phase 1 and up to 20.1 percent at 48 weeks in Phase 2, well above petrelintide’s ceiling. William Blair analysts called ZUPREME-2’s weight loss “intriguing” and noted the placebo-adjusted 7.2 percent matched what Lilly reported in Phase 2, but the gap widens at longer durations. The trial excluded nobody on background diabetes therapy, and weight loss is typically lower in people with type 2 diabetes, which helps explain part of the difference.
Roche paid $1.65 billion upfront for petrelintide in 2025, so the bar was high. The companies still plan to advance the drug into a comprehensive Phase 3 program. The bet now is that best-in-class tolerability and a monthly or less frequent dosing story can win share even if peak efficacy lags Lilly and Novo Nordisk, whose CagriSema combines amylin with GLP-1. That is a defensible niche in a market where discontinuation rates on GLP-1s remain a commercial problem, but it is not the efficacy leadership the price tag implied.
