Biomea Fusion’s first clinical test of BMF-650, its oral small molecule GLP-1 receptor agonist, delivered what a Phase 1 study is supposed to deliver: dose dependent weight loss and tolerability clean enough to support a simple dosing regimen. The market wanted more than that, and sold the shares on the October 13 readout, in a week when a better funded rival, Verdiva, walked away from a mid stage oral GLP-1 program on commercial grounds altogether.
Across the first four multiple ascending dose cohorts, mean weight reduction at day 29 ranged from 1.5 to 3.2 percent, against 0.1 percent for pooled placebo. A fifth cohort, kept in a confined setting and titrated rapidly in one step to the 400 milligram daily target dose, lost 6.1 percent, a figure the company and analysts both treat cautiously because of the confined setting. There were no severe or serious treatment emergent adverse events, no dose changes and no discontinuations for side effects across the treated participants. In the high dose cohort, nausea and vomiting were each reported by 12.5 percent of participants, low rates for this drug class, which underpins the company’s argument that BMF-650 may not need the long titration schedules that limit persistence on current GLP-1 medicines.
Management’s own framing points at the real constraint. Biomea said the findings warrant further evaluation including through potential collaborations, and reminded investors that its primary development focus remains icovamenib, its menin inhibitor in type 1 and type 2 diabetes. In other words, BMF-650 is being positioned as a partnering asset, not a program Biomea intends to carry into the large, expensive trials an obesity drug requires.
Set against oral Wegovy on the market and Lilly’s oral program advancing, a four week, 40 patient data set with a best outpatient cohort at 3.2 percent is a starting point, not a differentiator. The tolerability signal is the asset’s genuine claim. Whether anyone pays for it will say as much about the partnering market for oral GLP-1s as about the drug.
