Fractyl Health announced a restructuring that cuts 45 percent of its workforce and ends internal development of its Rejuva gene therapy platform, concentrating the company on a planned Food and Drug Administration De Novo filing for Revita late in the fourth quarter of 2026. The company said the cuts extend its cash runway into the second quarter of 2027 and should roughly halve quarterly operating cash burn to about 8 million dollars by then. The announcement followed, by days, the data it was meant to celebrate.
On October 16, Fractyl reported that Revita, an endoscopic procedure that resurfaces the lining of the duodenum, met the six-month co-primary endpoint of the pivotal REMAIN-1 study. In 308 United States adults with obesity who had lost at least 15 percent of body weight on tirzepatide and then stopped it, Revita patients regained an average of 5.1 percent of body weight at six months against 8.2 percent on a sham procedure (p=0.0023), retaining about 80 percent of their GLP-1-induced weight loss against about 67 percent for sham. The company reported no device- or procedure-related serious adverse events.
The market response was the opposite of a data rally. Shares fell about 63 percent on the announcement, from 4.16 dollars at the October 15 close to about 1.15 dollars, MedCity News reported, as investors weighed results that were positive but below analyst expectations, a dilutive equity financing registered around the readout, and securities litigation over earlier trial disclosures. Fractyl’s market value is now roughly 15 million dollars.
The clinical question is only half answered. REMAIN-1’s second co-primary endpoint, at least 5 percent weight loss maintained at 12 months in more than 50 percent of patients, reads out in the first quarter of 2027, after the planned filing. Revita already holds a Category III CPT code, 1122T, effective January 1, 2027. A procedure that durably holds even part of GLP-1 weight loss would address the central weakness of the obesity drugs, regain after stopping, but Fractyl now has to reach that readout as a single-asset company.
