Insmed raises Brinsupri guidance after blockbuster first year

Life Sciences Sunday, November 1, 2026 - Updated Nov 1, 2026 07:15

Insmed reported third quarter revenue of 524.4 million dollars, up 268 percent year over year, with Brinsupri at 400.1 million dollars in the quarter and more than 1 billion dollars in its first four full quarters, and raised 2026 Brinsupri guidance to 1.35 billion to 1.4 billion dollars.

Why it matters: A first in class bronchiectasis drug reaching blockbuster scale in a year, with a Japan launch under way, is the clearest current example of a rare disease launch outperforming its own plan.

Data as of Insmed third quarter 2026 release of October 29, 2026, for the quarter ended September 30, 2026; revenue and guidance figures are the company's own.

Insmed reported third quarter revenue of 524.4 million dollars, up 268 percent from 142.3 million dollars a year earlier, driven almost entirely by Brinsupri (brensocatib), its treatment for non-cystic fibrosis bronchiectasis. Brinsupri revenue was 400.1 million dollars in the quarter, up 29 percent from the second quarter, and the company said the product passed 1 billion dollars of revenue in its first four full quarters on the market.

The older product also grew. Arikayce revenue was 124.4 million dollars, up 9 percent year over year, helped by international markets. The company still lost money: the net loss was 100.3 million dollars, or 0.46 dollars per share, narrower than the 370.0 million dollar loss a year earlier but wider than the consensus loss estimate, even as revenue beat expectations. Cash, cash equivalents and short term investments were about 1.1 billion dollars at September 30.

Management raised the low end of guidance for both products. Full year 2026 Brinsupri guidance is now 1.35 billion to 1.4 billion dollars, from 1.25 billion to 1.4 billion dollars previously, and Arikayce guidance is now 460 million to 470 million dollars, from 450 million to 470 million dollars. The company continues to guide to peak revenue of more than 7 billion dollars for Brinsupri, more than 6 billion dollars for its TPIP pipeline programme and more than 1 billion dollars for Arikayce; those are company estimates, not results.

Regionally, Japan’s Ministry of Health, Labour and Welfare approved Brinsupri tablets 25 mg for non-cystic fibrosis bronchiectasis in adults and patients aged 12 and older, and Insmed launched the product in Japan in October 2026. Management expects Japan to be the main contributor to Brinsupri revenue outside the United States.
