FDA approves Sun Pharma Ilumya for psoriatic arthritis

Life Sciences Sunday, November 1, 2026 - Updated Nov 1, 2026 07:15

The FDA approved Sun Pharma's Ilumya (tildrakizumab-asmn) for adults with active psoriatic arthritis on October 30, adding a second disease to a label first cleared for plaque psoriasis in 2018.

Why it matters: Ilumya enters a crowded IL-23 class against AbbVie's Skyrizi and Johnson and Johnson's Tremfya, differentiated only by provider administration, so uptake will test whether in-office dosing is a feature or a friction.

Data as of FDA approval announced October 30, 2026 in the Sun Pharma release, read in full in this run; the release gives no ACR20 response rates, and none are stated here.

The Food and Drug Administration has approved Sun Pharma’s Ilumya (tildrakizumab-asmn) for adults with active psoriatic arthritis, the company announced on October 30. The decision clears a supplemental biologics license application and adds a second disease to a label the FDA first approved in 2018 for adults with moderate to severe plaque psoriasis who are candidates for systemic therapy or phototherapy.

Ilumya is a humanized monoclonal antibody that binds the p19 subunit of interleukin-23. It is given as a 100 mg subcutaneous injection by a healthcare provider at weeks 0 and 4 and then every 12 weeks, which makes it the only IL-23 inhibitor in this class that is administered exclusively in the office rather than self-injected at home. Sun Pharma puts the United States psoriatic arthritis population at about 2.4 million people.

The approval rests on INSPIRE-1 and INSPIRE-2, two 52 week global Phase 3 studies that were randomized, double blind and placebo controlled. The company said both studies showed superiority over placebo on the primary endpoint, an ACR20 response at week 24, assessed after only two doses. Sun Pharma did not publish the response rates in the approval release, and the full dataset has not yet appeared in a peer reviewed journal, so the size of the treatment effect cannot be judged from the announcement alone. Safety was consistent with the established profile, with no new signals; the most common adverse reactions in psoriatic arthritis, at 3 percent or more, were upper respiratory tract infections and COVID-19.

The commercial question is positioning. Tildrakizumab becomes the third selective IL-23 p19 inhibitor approved for joint disease in the United States, behind guselkumab (Tremfya) and risankizumab (Skyrizi), both of which are self-administered and far larger franchises. Sun Pharma is presenting provider administration as the differentiator, aimed at patients who cannot self-administer or who benefit from regular in-office care. The release gave no launch timing or pricing.
