Immunocore combination shows early activity in uveal melanoma

Life Sciences Saturday, November 7, 2026 - Updated Nov 7, 2026 07:07

In a Phase 1/2 study, Immunocore's brenetafusp combined with Kimmtrak produced a 29 percent response rate and an estimated 70 percent two year survival rate in 17 evaluable first line patients with metastatic uveal melanoma.

Why it matters: The data are early, single arm and small, but they are the first showing two T cell engagers can be combined in a solid tumour, and Immunocore will now test the pairing in a randomised Phase 2/3 trial.

Data as of Phase 1/2 data (NCT04262466) presented at the SITC annual meeting on November 6, 2026, June 2026 data cutoff; company release read in full via syndication. Early single arm data in small cohorts, no randomised comparison.

Immunocore presented the first clinical data combining two of its T cell engager molecules at the Society for Immunotherapy of Cancer annual meeting on November 6. The Phase 1/2 trial enrolled HLA-A*02:01 positive patients with metastatic uveal melanoma. At the June 2026 data cutoff, 22 patients had received clinically active doses of brenetafusp alone and 26 had received brenetafusp combined with Kimmtrak (tebentafusp), the company’s approved therapy.

In the 17 evaluable patients treated first line with the combination, the overall response rate was 29 percent, the disease control rate was 71 percent and the median duration of response was 24.1 months. Median progression free survival was 8.1 months. Median overall survival had not been reached at a median follow up of 29 months, with estimated survival of 88 percent at one year and 70 percent at two years, and 10 patients remained on study. Circulating tumour DNA data, an exploratory measure, showed a molecular response in 8 of 9 evaluable combination patients, with clearance in 4.

The monotherapy arm provides the internal reference, though not a randomised one. Of 16 evaluable patients on brenetafusp alone, 3 responded, a 19 percent rate, with a median duration of response of 14.5 months, median progression free survival of 4.2 months and median overall survival of 20.3 months. The company described the combination’s safety profile as manageable, and no patient was reported to have discontinued for a treatment related adverse event in the summaries of the presentation.

The two molecules target different tumour antigens, Kimmtrak a gp100 peptide and brenetafusp a PRAME peptide, with PRAME an established marker of worse prognosis in this cancer. These are early, single arm results in 17 first line patients, and response and survival estimates at this size carry wide uncertainty. The test that matters is the one Immunocore announced alongside the data: DUET-UVEAL, a randomised Phase 2/3 trial of the combination in unresectable or metastatic uveal melanoma, for which no start date was given.
